Our Clinical Pharmacists
Clinical pharmacists have specialist training in the management of medications. They may see you in clinic or speak to you by telephone appointment to support you with your medications, as well as working behind the scenes to help with managing your prescriptions.
Their roles include:
- Supporting you in managing long-term conditions such as Diabetes, High Blood Pressure, and Mental Health problems
- Discussing your current medications with yourself, including any changes that may be needed
- Reviewing your long-term medications to make sure they continue to be effective

Our PAs
PAs (Physician Associates) are trained in providing care alongside practice GPs, including assistance in the assessment and management of various health conditions. They can help to monitor and manage your health conditions and symptoms.
Their roles include:
- Assessing and managing minor problems that do not need to be seen by your GP
- Assessing and managing long-term health conditions
- Requesting further testing or review by your GP or specialist services if your problem may be more serious

Our First Contact Physiotherapists
Physiotherapists (also known as “Physios”) are specially trained in the use of movement and exercise to treat a variety of health problems. They can help to manage pain with certain conditions such as Arthritis, and to improve your mobility and co-ordination.
Their roles include:
- Diagnosing specific joint and muscle conditions
- Teaching exercises to manage certain conditions that affect joints, muscles, and nerves
- Providing lifestyle advice to empower you to manage certain conditions at home
- Requesting specialist input when needed to manage certain problems

Our Care Co-ordinators
Care Co-ordinators support patients in the management of their health problems to ensure their care is organised and effective. They can help by supporting the communication between yourself and various healthcare services and in monitoring your health.
Their roles include:
- Assessing your specific healthcare needs
- Supporting the development of personalised care plans to best meet your needs
- Supporting communications, including helping you to set up appointments and understand your treatment

Our Health and Wellbeing Coaches
Your health goes beyond a list of diagnoses and medications. Health and Wellbeing Coaches provide support in these scenarios, to help you to develop the knowledge and skills to take an active role in looking after your health.
Their roles include:
- Helping you identify factors in your life that may have a negative effect on your health and overall wellbeing
- Helping you manage your health problems with lifestyle changes alongside your prescribed treatment
- Helping you to find resources and support groups in the community
